Advance care planning simply means making choices now regarding your future medical treatment. Many patients, their families, and loved ones find peace of mind and assurance knowing their wishes will be honored in the event of a medical crisis. Our vision is that everyone will receive care that honors personal values and goals at the end-of-life.
We inspire conversations about the care people want. We help the public make informed choices, and we provide teaching materials for health care organizations on how to discuss, record and honor people’s end-of-life choices.
