Webinar on state-funded supports for family caregivers – Dec. 10

Across the state, many patients are stuck in hospitals because the patients can’t access appropriate care in their communities. Support from family members can be an important part of getting patients back to their communities. The state has developed a number of innovative programs to support older adults and family caregivers. The Department of Social and Health Services is hosting a webcast for hospitals and providers on Dec. 10, 8:30 a.m. – 10:30 a.m. to provide information about these programs. These programs can help some patients stay in their homes or discharge from the hospital in a safe and timely manner. More information is available online.
