CCM is a Medicare program for patients with multiple chronic conditions. Through CCM, patients receive a care plan, personalized care goals and a clinician who checks in monthly to provide education, connect patients to Social Determinants of Health resources and note any health or medication needs.
Patients also gain access to a 24-hour nurse line. These resources can help your hospital reduce hospital readmissions and costs, improve your attribution and shared savings results and provide personalized, preventative care to your patients.
