A recently released white paper from WHS Preferred Partner ChartSpan uses 2023 claims data to demonstrate how chronic care management (CCM) can reduce costs and lower patient’s risk of hospitalization. The claims data shows that patients enrolled in CCM have 52% lower readmission rates, 36% lower inpatient costs and 21% lower overall health care costs. ChartSpan has developed a scalable, turnkey CCM program that offers continuous growth and benefits to both patients and health systems.
