The CMS’s Person and Family Engagement contractor is pleased to share the second edition of the PfP Strategic Vision Roadmap for Person and Family Engagement. The  goals for the second edition of the Roadmap were to:

  • Make the Roadmap more user-friendly for HIINs and hospitals, in large part, by integrating content from the two Addendums to the first edition of the Roadmap (January 2016)—Defining the Person and Family Engagement (PFE) Metrics for Improved Measurement: Purpose and Intention of the Five PFE Metrics and How Person and Family Engagement Can Help Hospitals Achieve Equity in Health Care Quality and Safety; and
  • Update the Roadmap so that it reflects the current landscape and language of PfP, including clarifications to the five PFE metrics based on feedback from HIINs and hospitals;
  • Add data on the impact of PFE on hospitals readmissions and hospital-acquired conditions (HACs); and
  • Add success stories to illustrate how hospitals are meeting each of the five PFE metrics.

The second edition of the Roadmap replaces the first edition and the PFE metrics addendum. Although the second edition of the Roadmap includes some information about the connection between PFE and health equity, we encourage hospitals to continue to refer to the health equity addendum for more in-depth information.
