The Washington Health Care Authority is seeking survey responses from primary care provider groups related to their capacity to adopt the Primary Care Transformation Model. Specifically, HCA needs to understand clinic’s/practice’s current capacities and needs as it designs a sustainable primary care model for patients, providers, payers, and purchasers. Additionally, HCA seeks to determine to what extent groups that are already providing whole person care as outlined in the model. WSHA strongly encourages members that provide primary care to respond the survey. We believe it is critical that HCA have a clear and comprehensive understanding of what is needed for the model to be successful, particularly for small or rural primary care providers that may lack community resources or are limited in their ability to assume financial risk. The model is designed to increase the proportion of resources for primary care and align quality metrics and payment incentives among payers, but also assumes transformation to a delivery model that requires greater coordination and infrastructure, with movement toward capitation and value-based payment.
